Youth Homes offers several programs for youth and families. Our online portal (BestNotes) allows you to submit applications directly for the services you need.
Use the guide below to determine which application(s) to complete.
– Therapeutic Group Care
Please submit both of the following via our online portal:

– Dan Fox Family Program Services
Start by completing the Dan Fox Care Programs Supplemental Application via our online portal.
After that, submit the application listed for the service you are requesting:
Outpatient Therapy: Complete the Standard Application
Home Support Services: Complete the Standard Application
Therapeutic Foster Care (TFC): Complete the TFC Youth Application
– Supervised Visitation & Family Based Services
PDF instructions for these services are available on the right-hand side of the portal under PDF Forms.
Important:
We only accept referrals from CFSD caseworkers for Supervised Visitation and do not accept private applications.
The PDF includes contact information for submitting the state 050 referral form.
– Transitional Living Services (TLS)
Instructions and forms are available on the right-hand side of the portal under PDF Forms.
The PDF provides contact information for submitting the DPHHS-CFS-134 (TLS referral) form.
Important Notes
Incomplete applications will not be reviewed.
To check the status of an application, go to Online Forms → Continue/Update Existing Application.
Any incomplete application will be clearly labeled.Be sure to click “Submit Form” at the end of a completed application.
