Make a Referral

Our goal is to help as many youth as we can, and referrals are how we connect with youth in need. Youth in our care must meet at least one of the following criteria.

  • Children must be placed by individuals or agencies with the authority to place the child and the resources to pay for the placement.
  • Children must also have health insurance (Medicaid or other coverage).
  • Referrals for our children most often come from Western Montana, but we are willing to consider placements from all other regions and communities in Montana.
  • Child Placing Agencies that access our services include Child Protective Services, Mental Health and Juvenile Parole, Tribal Social Services, and Youth Court Probation.
  • We also accept private pay.

Even if you do not fit within these categories but know of a child in need, please contact Charlie Wellenstein at cwellenstein@youthhomesmt.org to determine if we can help or direct you to someone who can.

Youth Homes offers several programs for youth and families. Our online portal lets you submit applications for the services you need directly. You can also make a referral application by downloading the .pdf files, or we can send you an application by mail.

If you are new to our referral system, please read the detailed instructions before visiting the portal.

Youth Homes offers several programs for youth and families. Our online portal (BestNotes) allows you to submit applications directly for the services you need.

Therapeutic Group Care

Please submit both of the following via our online portal:

  • Standard Application
  • Group Care Supplemental Application

Submit supporting documents by email to:

Dan Fox Family Program Services

Start by completing the Dan Fox Care Programs Supplemental Application via our online portal.
After that, submit the application listed for the service you are requesting:

  • Outpatient Therapy: Complete the Standard Application
  • Home Support Services: Complete the Standard Application
  • Therapeutic Foster Care (TFC): Complete the TFC Youth Application

Supervised Visitation and Family-Based Services
PDF instructions for these services are available on the right-hand side of the portal under PDF Forms.

Important:

We only accept referrals from CFSD caseworkers for Supervised Visitation and do not accept private applications.

The PDF includes contact information for submitting the state 050 referral form.

Transitional Living Services (TLS)

Instructions and forms are available on the right-hand side of the portal under PDF Forms.

The PDF provides contact information for submitting the DPHHS-CFS-134 (TLS referral) form.

Important Notes:

  • Incomplete applications will not be reviewed.
  • To check the status of an application, go to Online Forms → Continue/Update Existing Application.
    Any incomplete application will be clearly labeled.
  • Be sure to click “Submit Form” at the end of a completed application.
    You will receive an email confirmation once we have received your submission.
