They are age 16 or older and currently in state foster care; “aged out” of foster care and not yet 21 years old; were in foster care at age 18, or after their 16th birthday, have had a guardianship established or adoption finalized.
Complete Ansell Casey Life Skills Assessment yearly to use as a roadmap for support.
Create a Transitional Living Plan outlining goals every six months. Meet with Transitional Living Services (TLS) manager at least monthly to work on goals & address barriers.
High school graduation or GED completion incentive funds. Education Training Voucher – must be enrolled in post secondary school before age 21. Funds for tuition, housing, books and living expenses.
Career assessments, resume writing, job search, interview skills & job shadow.
Resources for finding affordable rental, saving for a deposit, navigating landlord/tenant responsibilities, home safety & household set up.
Assist, develop and maintain budget, set up bank account, balance checkbook.
Family Support & Marriage Counseling, Birth control & STD prevention, nutrition, self-care, mental health, healthy relationships & communication skills.
Acquiring documents – birth certificate, ID’s, drivers’ license, etc.
